Forms let you collect structured requests and turn each submission into a card on a Superthread board. They are useful for intake workflows like bug reports, access requests, content requests, design briefs, IT tickets, and anything else where you want people to submit consistent information. Each form belongs to a workspace, sends submissions to a destination board, and can be shared by public link or shown in the authenticated forms portal.

Create a form

Go to Settings > Workspace > Forms, then click Create form and choose the board where submissions should be created. When creating or editing a form, you can set:
  • Name and icon - shown on the form, in the forms portal, and in submissions.
  • Description - shown at the top of the public form.
  • Destination board - the board where submitted cards are created.
  • Folder - where the form appears in your workspace forms list and the forms portal.
  • Portal visibility - whether the form appears in the authenticated forms portal.
  • Fields - the questions and card properties people fill out.
New forms start with default Title and Description fields. You can rename, remove, reorder, or replace these fields.
If the destination board is deleted or unavailable, the form will show a warning in settings. Edit the form and choose a new destination board before using it again.

Supported field types

Forms support these field types:
  • Short text - one-line text, up to 500 characters.
  • Long text - longer text, up to 5,000 characters.
  • Number - accepts numeric values.
  • Date - lets the submitter choose a date.
  • Dropdown - choose from a list of options. Dropdowns can allow one option or multiple selections.
  • Attachments - upload one or more files with the submission.
  • Checkbox - a single checked/unchecked field.
  • Email - validates email addresses.
  • Phone - validates phone numbers.
  • Link - validates URLs and adds https:// when the submitter omits it.
  • Card property - maps a form answer directly to a card property.
  • Info block - display-only text for instructions, context, or links.
Every field can have a label and description. Short text and long text fields can also have placeholder text. Fields can be marked as required, except info blocks because they are not submitted.

Card property fields

Card property fields let a form set specific properties on the card created by the submission:
  • Priority
  • Assignees
  • Due date
  • Tags
Each card property can be added to a form once. Assignee options come from the destination space, and tag options include workspace tags and tags from the destination space.

Conditional fields

You can show a field only when another field matches a condition. Conditional fields can depend on:
  • Dropdown fields
  • Checkbox fields
  • Number fields
  • Date fields
  • Priority card property fields
Conditions can use all or any matching logic. Supported comparisons include equals, does not equal, contains for multi-select dropdowns, number comparisons, and date comparisons. When a conditional field is hidden, its value is cleared and it is not included in the submission.

Share a form

Each form can be shared with a public link. In Settings > Workspace > Forms, use the share control on a form row to turn the public link on, copy the URL, or regenerate the URL. Regenerating a form URL invalidates the previous public link. Use this when a link was shared more widely than intended. Anyone with the active public link can open and submit the form. Signed-in workspace members can also add participants while submitting, if participant selection is available for that form.

Forms portal

The forms portal gives signed-in users one place to find forms and track form submissions. It includes:
  • Chat - describe what you need, and Superthread can help choose or fill a form using AI credits.
  • Forms - browse portal-visible forms and folders.
  • Team submissions - view submissions from the selected workspace.
  • My submissions - view requests you submitted or that were shared with you.
Forms only appear in the portal when Show in forms portal is turned on for the form. Folders with portal-visible forms also appear in the portal sidebar.

Submit and track requests

When someone submits a form, Superthread creates a card on the form’s destination board. The card contains the submitted values, attachments, and any mapped card properties. In the portal, submissions can be searched and filtered by:
  • Text search
  • Status
  • Request type / form
  • Team submissions or submissions shared with you
Opening a submission shows the card status, submitted date, form name, due date, priority, assignees, participants, submission content, attachments, and conversation. Submitters and participants can use the portal conversation to comment on a request. Comments can include attachments and reactions.

Organize forms

Workspace forms can be organized into folders from Settings > Workspace > Forms. You can:
  • Create folders with names and icons.
  • Drag folders into a custom order.
  • Drag forms within a folder or between folders.
  • Leave forms uncategorized.
  • Rename or delete folders.
Deleting a folder does not delete the forms inside it. Those forms move back to the uncategorized section.

Permissions

Guests cannot manage workspace forms. People who can manage forms can create, edit, duplicate, delete, reorder, publish, unpublish, and regenerate form links. The forms portal requires sign-in. Public form links can be opened directly by anyone with the active link.
